Insurance Certificate Checklist

What your agent should provide us with on their certificate of insurance or Acord form (if available).

The Lender

First Western Bank & Trust, DBA: First Western Equipment Finance, ATIMA, 100 Prairie Center Drive, Eden Prairie, MN 55344 as certificate holder, loss payee or additional insured, depending on your finance or lease contract.

Contract (agreement) Number

This is located on your finance documents from First Western Equipment Finance on the first page.

Cost of Equipment

The total cost of the equipment is typically on the dealer's invoice. This is the cost of the actual equipment, not the total amount of the loan.

Policy Dates

The effective and expiration dates of the policy. Just like your homeowner or auto insurance.

Insurance Provider

The name of your insurance provider (or carrier) and your policy number.

Some of the standard questions from our customers.

When does the insurance certificate need to be in to First Western Equipment Finance?

We prefer that the certificate be sent in with all other documents sent to the account manager. If that isn't possible, as soon as possible thereafter, to make sure that customers are protected from loss or damage.

Will First Western Equipment Finance contact the agent?

We will follow-up with the agent to obtain the correct documents, but the customer must make an initial contact with their agent to add the equipment to their policy.
